Hip & Knee Pain in Sarasota
In the journey of life, every step should be a testament to your resilience, not a reminder of discomfort. At Manual Physical Therapy & Concierge Services, we understand the silent stories your steps narrate.
What causes hip and knee pain?
Hip and knee pain is rarely just a hip or knee problem. When you live with pain, you shift your weight to protect yourself. Over time your pelvis rotates and your hips shift to one side, which puts uneven stress on one hip or knee. That's why it is often one knee and the opposite shoulder. Treating only where it hurts leaves the real problem in place.

Pain around the kneecap gets blamed on the knee itself. But the knee follows what happens above and below it. If your pelvis is rotated or one foot turns out more than the other, the knee twists a little with every step. Do that thousands of times a day and the cartilage gets irritated. Correct the rotation and the knee often settles.
Bursitis and labral irritation are real, but they are often the trigger, not the whole story. When your hips shift to one side, that hip carries more load every time you stand or walk. The irritation keeps returning because the shift is still there. Correct the shift and the hip is no longer overloaded day after day.
Pain on the outside of the knee or hip often gets labeled IT band syndrome. The IT band is part of your fascia, which connects you from head to toe like skin. When years of compensation twist that fascia, the outside of the leg is pulled tight. Stretching and foam rolling only go so far. Untwisting the fascia often brings a noticeable difference right away.
Many people are told they are bone on bone. Arthritis can be the trigger, but it doesn't explain why one knee hurts and not the other. Think of a car with misaligned wheels. The tires wear unevenly. An asymmetrical body wears out some joints faster than others. Restoring symmetry can ease pain even when imaging shows arthritis.
Sit down and cross your legs one way, then the other. If one knee sits higher, that isn't your knee. It's the position of your pelvis. A rotated pelvis changes how each leg moves with every step. You may tip to one side, drift when you walk, or bump your walking partner. That uneven pattern wears on the hip and knee until they hurt.
The meniscus cushions the knee. When one leg takes more load than the other for years, that cushion wears faster on that side. Some tears do need surgery. Many people with meniscus wear, though, are really living with the compensation around it. If that isn't corrected, the knee keeps getting overloaded, even after surgery.

What to expect from your recovery.
Assessment
We look at you from head to toe, not just your hip or knee. Which knee sits higher when you cross your legs. Which way your hips shift. Whether one foot turns out. Whether you tip when you stand on one leg. Then we tie what we find to your symptoms.
Untwist and realign
In 60-minute, one-on-one sessions, we first untwist the fascia that years of compensation have twisted. Then we realign the muscles of your hips, legs, and feet, one at a time. Changes are often visible within the hour.
Maintain and get back to living
A home routine of about 10 minutes a day maintains the recalibration. It isn't about building strength. Then we work on how you walk, climb stairs, and move, so you can get back to the walks, golf, and travel you've been missing without paying for it the next day.
I started my therapy so I could ski this winter. I had extreme pain on my right knee. It hurt just walking, let alone skiing.
